I’ve been given a questionnaire to fill in by my customer

The health and safety questionnaire you have been given by your customer is so that they can prove that you are competent to do the work for them. They have a legal duty to check you are health and safety compliant and this is the easiest way to do so.

You will need to provide documents with the completed questionnaire. The documents you will need are:

  • Insurance Certificates – Employers Liability, Public Liability and Contractors all Risk
  • Accreditation Certificates – SMAS, CHAS, Constructionline, SafeContractor
  • A Health and Safety Policy
  • Risk Assessments and Method Statements for the work you are doing
  • Coshh assessments for any substances you will be using
  • Copies of training proof – certificates or cards
  • Accident statistics
  • Details of Health and Safety Executive Notices

The listed items are usually the minimum required. You may be asked for more information.

How to Fill in a Health and Safety Questionnaire

Answer the questions honestly. If you have had a HSE notice, declare it, because your company can be checked on the HSE Enforcement Database. Put all your employees on the training matrix – trained or not – because they may turn up on site and you will be found out. Your accident statistics can be justified with the actions taken to prevent the accident happening again.
